Every claim about what cold water does to a woman's body rates LOW, because the trials are tiny or absent. One claim rates HIGH: that the headline evidence base is effectively male-only. That is a fact about the literature, checked against the paper's own table, not an efficacy claim.

What would change this: a trial of at least 60 resistance-trained premenopausal women, randomised to cold water immersion or active recovery after every session of a supervised 10 to 12 week training block, with muscle fibre size measured directly by biopsy and cycle phase tracked by blood test rather than calendar counting.

What would change my mind on: cold water blunts muscle growth in women

Right now this is an extrapolation from four male trials, so it is LOW by default rather than by refutation. If the trial above showed the same drop in muscle fibre size that Roberts 2015 and Fyfe 2019 found in men, the male extrapolation would be validated and this would move to MODERATE for everyone.

If it showed no attenuation in women, the current blanket advice is simply wrong for half the population, and the likely explanation is already sitting in the literature: Solianik 2014 found men mount a significantly larger adrenaline and inflammation response to identical cold, and it is that response cold is thought to suppress.

What would change my mind on: cold water does not speed recovery in women

Two independent trials point the same way, which is why this is the strongest of the efficacy claims, but they total 58 women between them and one had only 10 per group. A properly powered trial of 100 or more women measuring strength recovery at 24, 48 and 72 hours would settle it either way.

A single well-run positive trial would not overturn it. A second null would move this to MODERATE.

What Most People Think

The common belief about ice baths

Ice baths are settled science. They help you recover, they are anti-inflammatory, and the one caveat everyone now repeats is that you should not take one straight after lifting because it blunts your gains.

There is also a widespread sense that the research has finally caught up on women, helped along by the 2025 systematic review that reports its participants as 55.4% female. Both halves of that are shakier than they look, and they fail in opposite directions.

What the Evidence Actually Shows

StrongThe "55.4% female" headline is technically true and completely misleading. HIGH Cain and colleagues pooled 11 studies and 3,177 participants. Ten of those eleven enrolled zero women. Every female participant came from one study, Buijze 2016, which contributed 3,018 people, roughly 95% of the pooled sample, and set the headline percentage single-handedly.

Participants across the other ten studies in that review, once the single cold-shower trial is removed. All of them male.

Worse for anyone reading it as ice bath evidence: the study supplying all the women was a cold shower trial, 10 to 12°C for 30 to 90 seconds daily for a month, measuring days off work. No exercise, no immersion, no recovery endpoint.

StrongNo woman has ever been enrolled in a cold water immersion training adaptation trial. HIGH The entire "ice baths blunt your gains" claim rests on Roberts 2015 (21 men), Fyfe 2019 (16 men), Earp 2019 (11 men) and D'Souza 2023 (9 men). Roberts found muscle mass and strength rose more with active recovery than with cold water, with type II fibre area up 17% and the number of nuclei inside each fibre up 26% in the active group, and neither reaching significance in the cold group. A real finding, about men, applied to everyone.

ModerateIn women, cold water does not appear to speed functional recovery. LOW Wellauer 2025 put 30 women through a drop-jump damage protocol and found that neither cold nor hot immersion beat simply sitting still on strength, swelling or soreness at 24, 48 or 72 hours. Hohenauer 2020 tested 28 women and found lower soreness with cold water but no effect on strength, swelling or jump performance.

ModerateThe physiology genuinely differs by sex, but not in the way people assume. MODERATE Solianik 2014 cooled 18 men and 14 women in 14°C water and found their cooling rates were identical. What differed was the method: women held heat in by insulating, men generated it by shivering. Men also showed significantly larger adrenaline and inflammation responses to the same cold dose. That last detail matters, because if cold blunts muscle adaptation by suppressing the post-exercise inflammatory signal, a smaller response in women is a plausible reason the effect could be smaller too. Nobody has tested it.

ModerateThe 40-year contradiction about who gets colder faster is a measurement artifact. MODERATE Tikuisis 2000 found women cooled at half the male rate. McArdle 1984 found women cooled faster than men. Both are right. Compare unmatched groups and women look protected, because they carry more fat under the skin. Match people for body fat and women cool faster, because a smaller body loses heat faster relative to its size. Sex is standing in for body composition here, not acting on its own.

The Debate

Does cold water actually reduce soreness in women?

Hohenauer 2020 · 28 women

Soreness significantly lower with cold water immersion than with no treatment, right through 72 hours. Cryotherapy did the same.

VS

Wellauer 2025 · 30 women

No soreness benefit at all versus simply sitting still. Hot water did nothing either.

Same damage protocol, same 10 minute immersion, so the dose is not the explanation. Two things likely are. Neither trial could be blinded, because nobody sits in 10°C water unaware of it, and Wellauer's own discussion points at earlier work where a lukewarm placebo bath produced recovery outcomes similar to real cold. Wellauer also had only 10 women per group, which is too few to detect a modest effect. Both trials agree on the harder endpoints: no effect on strength, swelling or jump performance.

Honest Limitations

A null result is not the same as no effect

The lab found: no recovery benefit from cold water in 30 women.
The complication: those 30 women were split into three groups of 10. That is far too small to see anything but a large effect, so "we could not detect it" is the honest reading, not "it does nothing".
Read it cautiously

The adaptation finding is borrowed, not measured

The lab found: cold water after lifting reduced muscle growth over 7 to 12 weeks.
The complication: every one of those participants was male. The mechanism hints the effect could be smaller in women, but that is a hypothesis with no trial behind it.
Stay conservative

The women studied are not the women asking

The lab found: results in women aged roughly 18 to 35, averaging 23.
The complication: the loudest popular claims are about perimenopausal symptom relief, an age group no trial here touched. The evidence there is a survey, not an experiment.
Do not transfer

The Nuance

Nuances the simple answer misses

Neither female trial could be blinded, and one of them says so. Nobody sits in 10°C water unaware of it. Some of the soreness benefit people report may be belief doing the work, which is not nothing, but it is not temperature.

The colder number is not the bigger dose. Partial-body cryotherapy at -135°C produced smaller physiological effects than 10°C water in the very same female trial, because water strips heat off skin far faster than air does. The chamber sounds more extreme and delivers less.

The perimenopause evidence is the weakest thing on this page and the most confidently repeated. Pound 2024 surveyed 1,114 women who swim in cold water and found many reported better anxiety, mood and hot flushes. There was no control group, and 56% to 63% of respondents said they were swimming specifically to relieve those symptoms. That is expectancy built into the design.

None of this says cold water is useless. It says the evidence for what it does to a woman's body is thin enough that anyone speaking confidently about it is filling in gaps with men's data.
